Which medical do I need?

HGV / Lorry (C, C1, C+E)
A D4 medical is needed when you first apply for a vocational licence, at 45, and every 5 years until 65. After 65, it's yearly.

The same Group 2 standard as HGV. Required to get and renew your PCV entitlement.

Most councils ask for a Group 2 medical when you apply or renew your badge. Check with your council if unsure.

What to bring and what to expect
Bring with you
- Photo ID and your driving licence
- Your D4 form if you have it (free to download from GOV.UK), or your council's taxi medical form
- Glasses or contact lenses you drive in
- A list of any medication you take
- Recent readings or letters if you have diabetes or another long-term condition
On the day
- A short chat about your health, plus your eyesight and blood pressure checked
- Your form completed and signed by the doctor before you leave
- You then send it to the DVLA or your council

What's a D4 medical?
It's the DVLA medical form for drivers of lorries and buses. A doctor completes it after checking your health and eyesight.
How often do I need one?
For HGV and bus licences, at 45 and then every 5 years until 65. After that it's every year.
Will I pass?
Most drivers do. If a condition needs more information, we'll explain what's needed so you know your next step.
Do I pay the DVLA or council separately?
Yes. The medical fee covers our appointment. Licence or badge application fees are paid to the DVLA or your council.
Can my normal GP do my taxi or HGV medical?
Not usually. Taxi, HGV and bus medicals must meet the DVLA Group 2 standard on the right form, so an ordinary NHS health check isn't accepted.
How long is my D4 medical valid?
Send it to the DVLA within 4 months of the doctor signing it, so book close to when you apply or renew.
My council has its own taxi form. Can you do it?
Yes, bring your council's form with you or message us the council name beforehand and we'll confirm.
Do you visit me at home?
Yes, we offer home visits. It helps if you cannot travel or your blood pressure rises at clinics. Message us for availability and price.
